Get the insurance. I have it. I've gone through at least 15 tires in 21/2 yrs.....YEP! No questions asked. Even bent 3 rims.......they replaced them too. I have the insurance for 4 years. 19 inch runflats don't do well in NYC. Once you leave NYC its pretty smooth sailing but potholes are everywhere, just drive safe. When the insurance period is up I plan to go to 18 or 17 inch wheels!
