Problems after retrofitted OEM backup camera

Hi,
I have retrofitted an OEM rearview camera in my F11 LCI 2014 built aug 2013.

I installed everything according to BMWs original instructions, I have no doubts on the installation itself, apart from possibly the cables into the IHKA, see below for details.

After the installation was complete, I coded the system with Esys and enet-cable
1. Added 3AG to FA
2. coded this FA to HU-NBT and PDC ECUs
3. Added CAFD (tried both the first one and the last one in the list, as it had no CAFD at first) to TRSVC ECU, and coded it.

Now the car has a working camera, however I think I have two problems. I'd be happy if someone could provide some ideas how to proceed.

A. When I activate the camera, the display under the RPM-meter shows "rearview camera failure"
B. Im not sure that the picture quality is as good as it should be, see image. see image (sorry for upside down pic, dunno why tinypic did that)

does it look about right for a quality perspective. The image is from a well lit shopping mall garage.

Here are some things I'm thinking about:
1. Have I done something wrong with the coding?
2. Is the retrofitted control module for the camera to new? i bought it used, a note on it says it comes from a donor car F23 (2-series cab) 11.2014.
the module says HW:10.02.00 SW 13.09.05
3. Is there a hardware error with the camera?
4. The cable between the camera and control unit, on the middle of it, it has a metallic ring, which i assume is for grounding, I tried grounding this but the error(s) remained. IO assume the seller of the package hooked me up with a cheaper cable set than the original one for F10/11

5. The wires that twisted and connects into IHKA control panel, there is bit of that I untwisted in order to fit the miniture connectors that connect to the existing cables.
6. Did I manage to get a good connection on the cables into the IHKA

Sorry for a long post, hope someone can give som guidance.

What pins did you use behind the NBT connector?

Thanks for response.
I connected according to installation instructions:
+ (yellow on my cable, but red on 5-series cable) to pin 21
- (white) to pin 24
shield (transparent) to pin 23

I'm considering getting hold of ista to read the code, or to flash the ECU of the controller unit for the camera.
